A policy changed. How do you show who read the right version?

Record the policy version, who needs the change, what they need to do differently and how you will check understanding or application. Keep acknowledgement, a knowledge check and a supervisor’s review as separate evidence.

Explain the change before assigning reading

Write a short release note in ordinary language: what changed, why it matters, who it affects and when the new process begins. Point to the exact approved version. If a form, checklist or participant-facing explanation also changes, include it in the rollout plan. Avoid a vague instruction to “read the updated policy” with no indication of the practical difference.

Choose the audience by role and work, rather than sending every update to everyone. Decide how to reach absent workers, casual staff and people who need another accessible format. Keep a responsible person for unanswered questions and exceptions. The release note should make the next action clear without concealing the complete document.

Separate three different kinds of evidence

Acknowledgement records that a person made a particular statement about a specified resource. A knowledge check records answers to questions. A supervised observation records a reviewer’s account of application in a defined situation. None automatically proves every other kind of evidence, and a completion certificate is not automatically a qualification or proof of practical competence.

Choose the checks that suit the change. A corrected telephone number may call for a different response from a new high-risk procedure. Have appropriate people determine the training or competency requirements. Keep the exact wording of an acknowledgement, the version involved and the actual person and time; avoid shared accounts that make authorship unclear.

Try a fictional rollout with an exception

In a fictional provider, the handover procedure changes from version one to version two. The new version asks workers to name an owner for unfinished tasks. The rollout note explains the change, links the current form and asks affected workers to identify the next owner in an example scenario. A supervisor later checks selected handovers.

One worker is away when the update is issued. Record the assignment as outstanding and plan follow-up before the worker resumes the affected task. Do not mark it complete because the rest of the team finished. If the form changes again, assess whether fresh reading or instruction is required; an earlier acknowledgement should not silently become evidence for different wording.

Keep the history understandable

Retain a small index: old version, new version, change reason, audience, assignment date, response, follow-up and reviewer. Preserve old evidence rather than overwriting it with the current version. If someone challenges what they were asked to read, you should be able to identify the resource that was actually assigned.
